Our disease management experts help identify, educate, monitor and mentor patients living with diseases such as: diabetes, heart disease, Parkinson’s, multiple sclerosis and COPD.

Enhanced Care includes:

Assisting bed bound clients with ADL's and preventing pressure sores.

Care and support for paraplegic or quadraplegic individuals.

Transfers with assistive devices such as a Hoyer lift, sit-to-stand, or slide board.

Providing support and assistance for cancer patients undergoing chemotherapy or radiation.

Managing a chronic condition to prevent further decline.

Staffing support for hospice or palliative care.

Setting up the home care environment and assistance with ordering appropriate equipment and supplies.
